ACCESS FEE
-Our annual access fee is $500 per patient, per year.
-Children of members between the ages of 17-26 do not pay an access fee.
-The access fee does not cover medical services provided by health insurance plans. We will continue to submit claims through your insurance carrier in the traditional manner.
-We are more than happy to work with you on payment plan options.
INSURANCE
-We will continue to submit claims through your insurance carrier in the traditional manner.
-We will accept most major insurance carriers including Medicare and Medicare Advantage Plans.
-The office will require your copay at time of service
-Please understand, it is your responsibility for any deductibles, coinsurance and other out-of-pocket expenses.
-You may be responsible for any bill or portion of a bill that is not paid by your insurance company.
-We will make every effort to provide services within the scope of your insurance policy.
If you are a self-pay patient or choose not to use your insurance for the visit, we will discount the visit if paid in full at time of visit.
Florida Snowbirds: We will be licensed in Florida for telehealth visits so that we can treat our patients in if the need arises.
Once our patient panel is full, we will start a waiting list in order to accommodate additional patients.
